Insurance

Accepted Health Plans:

  • Aetna

  • Blue Cross Blue Shield Healthplus (Medicaid)

  • Cigna

  • Healthfirst (commercial & Medicaid)

  • Northwell Health

  • Optum

  • Oxford

  • United Healthcare

For all other insurance companies, I am an out of network provider.  For these cases, you will need to pay at the time the service is rendered.  Please note that I do not submit to secondary insurance.  I will provide you with the documentation that you will need to submit claims for each visit to your insurance.  If you have questions regarding insurance matters, please contact us at (516) 307-0850 or cyndee.dunn@newleafwellness.org.

Self Pay Fee Schedule

Please note that the fee schedule is as follows:

  • Initial evaluation session - $150

  • 60 minute therapy session - $125

 

* Sliding scale fee is available on a limited basis.  Please inquire for more information.

Payment

I accept cash, checks, credit cards and Health Saving Account (HSA) cards.  Receipt will be provided via email. 

 

Checks should be made out to New Leaf Wellness Center.  You may also pay your balance through the Client Portal.

Financial Policies

The fee for a returned check is $10.  This is what the bank charges, so for that reason the cost is passed on to you.

Payment is due at the time of your appointment.  If you are having difficulty paying, let's please discuss this.  If I am are aware of a problem, I may be able to help you manage it or set up a temporary payment plan.

Cancelled appointments with less than 24 hours notice will be billed the $60 late cancellation fee.  

 

Missed appointments will be billed a $100 no call/no show fee.  As insurance does not reimburse for missed appointment, you will be fully responsible for the fee incurred.  For this reason, I hope that you will give adequate notice if you cannot attend your appointment.
